What is a Comprehensive Substance Use Evaluation?
A Comprehensive Substance Use Evaluation is an independent clinical assessment of an individual’s alcohol and/or substance use history, current functioning, and other relevant behavioral health factors.
The evaluation may include a review of substance use patterns, related consequences, treatment or recovery history, mental health and medical factors, available records, and other information relevant to the reason for the evaluation.
Unlike ongoing therapy, the purpose of an evaluation is to provide an objective clinical assessment and, when appropriate, diagnostic impressions and recommendations based on the information available.
Comprehensive Substance Use Evaluation | $350
The standard evaluation fee includes review of completed intake materials, a 90-minute virtual clinical evaluation, clinical assessment and formulation, diagnostic impressions when clinically appropriate, recommendations, and routine evaluation documentation.
A written evaluation report is included when applicable to the purpose of the evaluation.
Comprehensive Substance Use Evaluations are offered as a self-pay service and are not billed to insurance.
Fees & What’s Included
Some evaluations may require services beyond the standard evaluation, such as extensive record review, collateral interviews, additional appointments, consultation with attorneys or other professionals, or completion of specialized or extensive agency documentation. Additional fees may apply when these services are required and will be discussed before additional work is completed.
Because requirements vary among courts, agencies, states, licensing authorities, employers, and other organizations, evaluation requests are reviewed before scheduling to determine whether Centerline Clinical Group can appropriately provide the requested service.
